Formstack is a serious enterprise form and workflow platform, and it is aimed higher than most of the tools it gets compared against. It says it serves more than 32,000 organizations, it ships over 300 integrations on both self-serve plans, and it sells three products that fit together: Forms for collecting data, Documents for generating paperwork from that data, and Sign for eSignatures with no per envelope fees. The Suite plan bundles all three. There is a genuine Salesforce story too, with native Salesforce forms and document generation, which is the reason a lot of larger teams end up on Formstack rather than a cheaper builder. Two things make it an awkward fit for professional services intake. The first is price: at $83 a month billed annually, Formstack Forms is roughly two and a half times Jotform Bronze and nearly three times Typeform Basic or IntakeQ entry tier, and what you are buying with that premium is workflow and Salesforce depth rather than better intake. The second is that a form is still a form. It cannot ask a follow-up when an answer is vague, it cannot decline a matter you do not take, and it cannot book a paid appointment off a screening it never ran. HIPAA is only described as included on the Salesforce and Enterprise plans, and both of those are quote only. So there is no published price at which a Formstack buyer can collect protected health information. The right Formstack alternative depends on the limit you hit: IntakeQ includes a business associate agreement on every plan from $29.90 a month, Jotform gates HIPAA to Gold at $129, and Consultations runs the intake conversation itself, screens the lead, collects documents and books the paid appointment. Consultations launch pricing is a planned $49 to $299 a month per firm and is not purchasable yet.
Formstack wins on workflow depth and on Salesforce. If you need forms, document generation and eSignature stitched into one approval flow, or native forms living inside Salesforce, it is a genuinely strong platform and the 300 plus integrations back that up. Consultations wins when the job is the front door rather than the paperwork: when the person filling in the form is a stranger who has to be screened, when the answers need follow-up questions, when documents must be chased, and when the outcome you want is a qualified, booked, paid appointment. Formstack meters builder users and submissions per form and sells HIPAA as an unpriced add-on on both self-serve plans; Consultations plans to charge a flat $49 to $299 a month per firm.

Suite is $299 a month, or $250 paid annually, and includes 3 builder users, 100 forms, 5,000 submissions per form, 10GB of storage, 250 document deliveries a month and unlimited eSignatures. Both were read from the Formstack pricing page on 29 August 2026.
Then there is the feature comparison further down the same page. The row asking whether a plan can match HIPAA and compliance requirements returns Add On against both Forms and Suite. Formstack does not print a price for that add-on anywhere on the pricing page. HIPAA appears as included only in the Salesforce Solutions bullet list, which describes PCI and HIPAA compliant solutions, and by implication in Enterprise, and both of those plans read Custom Pricing with a Talk to Our Team button. The practical result is that a therapy practice, a clinic or a law firm handling medical records cannot work out from the Formstack website what compliance will cost them.
That is worth stating plainly because it is the strictest version of a pattern across this whole category. Jotform at least publishes the number: HIPAA is gated to its Gold plan at $129 a month, or $99 billed yearly, read from Jotform own plan data on 26 August 2026. IntakeQ goes the other way and includes a business associate agreement on every plan, starting at $29.90 a month on its low volume tier. Formstack sits at the opaque end, where the answer is an add-on with no rate and a sales call.

Submissions per form is a third way to meter a form builder, and it changes the maths
Read the Formstack allowance carefully, because the wording is unusual. Forms includes 1,000 submissions per form, and Suite includes 5,000 submissions per form. Per form, not per account. That is a genuinely different meter from the two you meet everywhere else: Jotform counts submissions a month across the whole account, and Typeform counts responses a month across the account. IntakeQ is different again and charges per practitioner.
Per form metering is generous in one direction and punishing in the other. A practice running three intake forms on a Formstack Forms plan has a theoretical ceiling of 3,000 submissions, and one running all 25 permitted forms has 25,000, which makes the headline price look much better per record than the entry figure suggests. But it also means the limit you actually hit is the form count and the single builder user, not the submissions. One person can build forms on the Forms plan. If two people at your firm need to edit intake forms, the plan does not fit no matter how few submissions you take.
This is the reason a straight price comparison across form builders is close to meaningless. Four vendors in this market meter four different things, so the cheapest headline is routinely the most expensive plan for a given practice. Work out which limit your actual workload hits first, then price only that.

The Formstack annual discount is exactly two months, and it does not scale with the plan
Formstack states the offer on the pricing page as saving two months with annual plans, and the arithmetic backs it up on both tiers. Forms falls from $99 to $83, which is 16.2 percent. Suite falls from $299 to $250, which is 16.4 percent. That is effectively ten months paid for twelve, applied uniformly.
Uniform sounds unremarkable until you compare it. Jotform annual discount, read on 26 August 2026, runs 12.8 percent on Bronze, 20.4 percent on Silver and 23.3 percent on Gold, so the more you spend the more prepaying is rewarded and the annual decision genuinely has to be made per tier. Formstack removes that decision: if two months free is worth it on Forms, it is worth exactly the same on Suite. Whichever way you lean, you only have to reason about it once.
One clause is easy to miss and worth reading before you assume discounts stack. Formstack own pricing FAQ states that its nonprofit discount applies to invoices but the annual discount does not. A nonprofit taking the nonprofit rate should therefore confirm in writing whether it can also take the two months, rather than budgeting for both.
When Formstack is the right choice
If your requirement is a form that feeds an approval workflow, generates a document from the collected data and comes back signed, Formstack does that as one product and does it well. Suite bundling Forms, Documents and Sign with unlimited eSignatures and no per envelope fees is a real advantage over stitching three vendors together, and per envelope pricing is exactly where eSignature bills quietly get out of hand. If your team lives in Salesforce, the native forms and document generation are a stronger argument still, and no general purpose form builder matches them.
The case for something else is narrower and specific. It is that a form is the wrong instrument for a first conversation with a stranger. A new client inquiry is not a data collection problem, it is a screening problem: you need to find out what happened, whether it is something you handle, whether the timing works, what documents exist, and whether this person should be given an appointment at all. A form captures whatever gets typed and stops. Nobody is screened, nothing is chased, and the qualifying still lands on your desk.
Firms often run both, and that is a sensible outcome rather than a compromise. Keep Formstack for the internal workflows, the document generation and the signature chain. Put a conversational intake on the new business front door, where the value of getting it right is a qualified, booked and paid appointment rather than a row in a submissions table.
